In-Home Outpatient Physical Therapist - PRN - South Bay & Long Beach
Torrance, CA · On the road · $80–$95 per visit
HealthSpark is hiring physical therapists to treat Medicare Part B patients in their homes across South Bay & Long Beach: Torrance, Redondo Beach, Manhattan Beach, Hermosa Beach, Gardena, Carson, Long Beach. Visits are available throughout LA County. Our team runs the back office so you can focus on the visits.
About the role
- Evaluate and treat Medicare Part B patients in their homes with outpatient physical therapy
- Set each patient's plan of care and goals, and see them through to discharge
- Work with patients' families and caregivers, including caregiver training sessions
- Pick up as many or as few visits as you want
Your patients
- We refer patients to you from physicians, home health agencies, and families in your area
- Or bring your own patients and treat them through HealthSpark
Pay
- $95 per evaluation
- $80 per follow-up visit
- Payouts twice a month on a fixed schedule
We handle the billing and back office
- Credentialing and enrollment with Medicare, Medicare Advantage, and commercial plans
- Eligibility and benefits checked before every visit, including Home Health episodes and Medicare Advantage plans
- Claims to Medicare, secondary and supplement plans, and patient balances
- Denials fixed and resubmitted until they're paid
- Every payment matched to its visit
- Plans of care faxed to the physician, with the signed copy filed in the chart
- Reminders for progress notes and recertifications
- Scheduling, intake, and patient outreach

Requirements
- Active California Physical Therapist license
- Thorough, defensible documentation that supports medical necessity
- Knowledge of Medicare Part B requirements, including plans of care, progress notes, and recertifications
- Experience in home health or mobile outpatient physical therapy
- Able to pass a background check
- Comfortable working as an independent contractor (1099)
- Reliable transportation within Los Angeles
