I'm a physical therapist with 7 years of experience specializing in general orthopedics rehabilitation. I received my DPT from the University of Saint Augustine for Health Sciences and I am passionate about helping patients regain lost function, get stronger after injuries and understanding their bodies to get back to doing what they love. I have a strong background in exercise, collegiate track and field, volleyball and yoga. When I'm not in the clinic, you'll find me hiking, boxing, doing yoga and taking dance classes. As a former collegiate athlete and a PT who has gone through rehabilitation post ACL tear, I understand and try to incorporate tailored treatment plans that align with each patient's lifestyle and goals.

I’m a licensed physical therapist with 3 years of experience, and a proud graduate of California State University, Northridge. Through my mobile physical therapy practice, I provide personalized, one-on-one treatment in the comfort of your home or preferred space. I believe physical therapy should be convenient, effective, and built around real life—not rushed clinic visits. My name is Arek. I’m a Doctor of Physical Therapy who specializes in orthopedic and sports-related injuries, with experience working with both active individuals and high-level athletes. I earned my Doctorate in Physical Therapy from the University of Southern California in 2020 and completed advanced orthopedic (2022) and sports (2024) physical therapy residencies through Adventist Health Glendale. My approach focuses on identifying the why behind your pain and creating a highly individualized plan to help you move better, feel stronger, and return to the activities you enjoy. I value clear communication, hands-on care, and progressive exercise, and I believe physical therapy should be collaborative, empowering, and tailored to you, not a one-size-fits-all approach.

I have been a physical therapist for more than 17 years and graduated from Northern Illinois University with a Master’s in Physical Therapy. My career has been spent primarily in outpatient orthopedics and chronic pain care, where I focus on a comprehensive, whole-person approach to treatment. I’m passionate about helping patients reduce pain, restore movement, and return to the activities that matter most to them. I enjoy partnering with each patient to understand their goals and create a plan that supports both recovery and long-term health. Outside of the clinic, I enjoy spending time with family and friends, staying active in ministry, and taking advantage of all that San Diego has to offer.

I'm a physical therapist in practice since 2007 in diverse patient care settings. After graduating with a doctorate in physical therapy from Rockhurst University, I established solid clinical experience at the outpatient clinic of a trauma level 1 county hospital for 4 years. There I rotated annually through inpatient floors to assist in the ICU and cardiac care units, allowing a depth of understanding for the spectrum of patients needs from acute illness to home. From there, I dove further into work with under-served communities and moved with my family to the Dominican Republic. I worked collaboratively on curriculum revision and taught physical therapy at the local university, researched, provided clinical education and treated a wide variety of patients for 7 years. Now back in the US since 2018, I'm happy to bring my rich experience and bilingual Spanish skill set to treat patients in their homes and community settings. I am a certified clinical instructor and bring a deep concern for my patient's well-being to my care.

Physical therapy for spinal stenosis at home

HealthSpark connects patients across greater Los Angeles, Ventura County, and San Diego County with licensed physical therapists who come to their home for outpatient physical therapy for spinal stenosis, covered by Medicare Part B and major insurers. No homebound requirement.

What is spinal stenosis?

Spinal stenosis is a narrowing of the spaces in your spine that can put pressure on the spinal cord or the nerves that branch from it. It most commonly affects the lumbar spine (lower back), though it can also occur in the cervical spine (neck). The classic lumbar stenosis symptom is leg pain or cramping that comes on with walking and is relieved by sitting or bending forward (called 'neurogenic claudication'). It develops gradually and is most common in people over 60. PT can significantly improve function and quality of life — and many people avoid surgery.

Common symptoms

  • Leg pain, cramping, or heaviness with walking that's relieved by sitting
  • Numbness or tingling in the legs or feet
  • Lower back pain, especially with standing or walking
  • Symptoms that improve when you bend forward (leaning on a shopping cart helps)
  • Difficulty walking long distances
  • Weakness in the legs
  • Less commonly: bladder or bowel changes (a red flag — see a doctor immediately)
  • Symptoms that have come on gradually over months or years

What causes spinal stenosis?

Spinal stenosis is almost always caused by age-related changes in the spine — bone spurs from arthritis, thickened ligaments, bulging discs, or facet joint enlargement that all narrow the spinal canal. Less commonly it can be caused by a herniated disc, spinal injury, or congenital narrow canal. The narrowing itself doesn't always cause symptoms — it depends on whether nerves are actually being compressed and inflamed.

How home visits help with spinal stenosis

Evaluation at home. The first visit is a full evaluation in the patient's home. The physical therapist asks about the history and goals, watches how the patient moves in the rooms they live in, and builds a plan of care.

Treatment where it happens. Visits follow that plan: hands-on treatment where it helps, and exercises taught with the furniture, stairs, and floors the patient uses every day.

The doctor stays in the loop. The plan of care goes to the patient's doctor, with progress notes as therapy continues, and follow-up visits run until the goals in the plan are met.

Frequently asked questions

Do you treat spinal stenosis at home?

Yes. Our licensed physical therapists treat spinal stenosis in the patient's home across greater Los Angeles, Ventura County, and San Diego County. Visits are outpatient physical therapy, covered by Medicare Part B and major insurers, with no homebound requirement.

Will I need surgery for spinal stenosis?

Most people don't. Research shows that for moderate stenosis, PT and exercise produce similar outcomes to surgery for most patients over 1–2 years. Surgery is typically reserved for severe cases with significant weakness, progressive symptoms, or quality-of-life impact that doesn't respond to conservative care.

Why does bending forward help my symptoms?

Bending forward (lumbar flexion) opens up the spinal canal and reduces pressure on the compressed nerves. That's why people with stenosis often feel better leaning on a shopping cart or pushing a stroller — and why flexion-based exercises are central to PT for this condition.

Can I still exercise with stenosis?

Yes — exercise is one of the most important parts of managing stenosis. The trick is choosing the right exercises (typically flexion-based, low-impact) and modifying around your symptoms. A stationary bike, swimming, and walking with sit breaks all work well for most patients.

Will my walking distance ever improve?

For most patients, yes. With consistent PT and a structured walking program (intervals with sit breaks to start), walking tolerance gradually improves. Many patients double or triple their pain-free walking distance over a few months.

Is in-home physical therapy covered by Medicare?

Yes. In-home visits are covered by Medicare Part B. We're also in-network with Blue Shield, Anthem, Cigna, UnitedHealthcare, Aetna, Humana, and other major plans, and we verify your benefits before the first visit so there are no surprise costs.

Will I see the same physical therapist each visit?

Yes. You're matched with a licensed physical therapist for your evaluation and every follow-up visit on the same plan of care.

Where do you visit?

Our physical therapists visit homes within about a 20-minute drive of where they're based, across greater Los Angeles. The best way to check is to get started and enter your ZIP code.

Is this the same as home health?

No. This is outpatient physical therapy that happens in your home, covered by Medicare Part B. Home health is a different Medicare benefit for people who are homebound, and it usually comes with nursing and other services. You don't need to be homebound to see us.

Can I see you while I'm getting home health care?

Not at the same time. Medicare doesn't cover outpatient Part B therapy while you're in a home health episode. Once your home health care ends, we can start. If you're not sure, ask us and we'll check.

Do I get to choose my physical therapist?

Yes. We have physical therapists across greater Los Angeles. You can learn about each one and request a visit with the one you want.

Do you accept my supplement plan?

Yes, we accept most Medicare supplement and secondary plans. When you book, we check your coverage in real time.

I've already reached the Medicare therapy limit. Can I still be seen?

Yes. Medicare covers therapy past that amount when it's medically necessary. Your physical therapist's evaluation and work with your doctor decide whether your care qualifies.

Can I pay cash?

For services Medicare doesn't cover, like wellness and fitness sessions, yes. Your physical therapist can talk through what that looks like, and we'll ask you to sign a Medicare notice (an ABN) before any service Medicare may not pay for.

Can you work with my family member or caregiver?

Yes. Your family member or caregiver can coordinate your care and be our go-to person. They can book for you on our page for caregivers.
